ASSERTIVE COMMUNITY TREATMENT (ACT)
Envision Health’s Assertive Community Treatment (ACT) program provides intensive, community-based behavioral health services for adults living with serious mental illness who need more support than traditional outpatient care can provide.
Our multidisciplinary team works with individuals where life actually happens, at home and in the community to help build stability, independence, and a better quality of life.

WHO CAN BENEFIT FROM ACT?
ACT may be appropriate for adults living with serious mental illness who experience challenges such as:
✓ Frequent psychiatric hospitalizations or crisis services
✓ Difficulty maintaining stability with traditional outpatient treatment
✓ Challenges consistently taking or managing medications
✓ Difficulty maintaining safe and stable housing
✓ Challenges completing everyday living activities independently
✓ Co-occurring mental health and substance use concerns
✓ Difficulty connecting with or remaining engaged in treatment
✓ Frequent involvement with emergency or community systems
✓ A need for intensive support to remain successfully in the community
ACT is designed for individuals with significant behavioral health needs who may benefit from a higher level of community-based support. Eligibility is determined through assessment and applicable program criteria.
WHAT DOES ACT HELP WITH?

Assessment, treatment planning, symptom management, counseling, and ongoing behavioral health support.

Medication education, monitoring, coordination, and assistance with maintaining prescribed treatment.

Help identifying warning signs, developing safety plans, preventing crises, and accessing appropriate services when additional support is needed.

Support with routines, organization, appointments, transportation planning, household responsibilities, and independent living.

Assistance identifying housing resources and developing the skills necessary to maintain stable housing.

Support identifying meaningful employment, educational opportunities, and personal goals.

Integrated support for individuals experiencing both serious mental illness and substance use concerns.
